It’s simple. All you need to do is complete a Choice of superannuation fund form with your details and hand it to your employer.
If you don’t have a Choice of superannuation fund form you can use ours. It’s attached to our Leaving your employer fact sheet. You just need to insert your details including your Mercy Super member number which you can find on any recent correspondence (or by contacting us).

If you’re starting work with Queensland Health you can arrange for your super contributions to be paid into your existing Mercy Super account – just like hundreds of other Mercy Super members. Queensland Health is one of our core participating employers which means your new employment details can be used as the basis for your Mercy Super insurance cover.
If you want to benefit from any increases in your insurance cover as a result of your new role with Queensland Health without any health evidence1, you’ll need to advise Queensland Health to pay your super to your Mercy Super account within 120 days of starting your new role.
To arrange for Queensland Health to pay your super into your existing Mercy Super account you’ll need to use the ATO’s Superannuation Standard choice form. We have a partially competed version with some helpful tips to help you on your way.
